Panthers News · Fall Tryouts Coming Soon

Good Afternoon,

With the first week of August already here, it’s time to start gearing up for the fall sports season. Fall Sports begin on Wednesday August 14th.  Below is everything you need to know to guarantee a smooth start to your student’s first days of tryouts.

All paperwork, including a scanned copy of your physical, should be submitted through the online portal found by clicking this link: https://app.formreleaf.com/organizations/parkdale-high-school/programs/20192020-sports-registration Here you can find directions, login information, and forms that need to be completed PRIOR to tryouts or your student will not be eligible to participate. You can also get to this page by going to the school website, hovering over “Athletics” and clicking “Athletic Forms”.

Below are several options still available for Pre-Participation Sports physicals being held in the area over the coming days at a discounted cost. Also attached is the physical and health history form for download/printing. Print only the necessary forms and take with you to your child’s physician regardless of where you go. This can also be found using the link above. Note that a well-child exam is not the same as a pre-participation sports exam. If you use any form other than the one attached or available on our website, the paperwork must reference being cleared for physical activity, sports, or a variation of that. Any questions regarding online paperwork should be directed to Ms. Radiance Jackson, the Athletic Trainer at rjackson@pivoths.com
